RELATING TO ALLOWING DOGS IN RESTAURANTS. Authorizes restaurant owners and managers, at their discretion to permit dogs inside the restaurant with conditions.

RELATING TO ANIMALS. Establishes a spay and neuter special fund and allows funds from an income tax check-off to be deposited into the special fund.

RELATING TO VACCINATIONS. Allows pharmacists, per a physician's prescription, to administer the influenza vaccine to persons aged fourteen to seventeen provided that the pharmacist has fulfilled certain requirements.

MAKING AN APPROPRIATION FOR EARLY CHILDHOOD HEALTH. Appropriates funds to the Department of Health to support early childhood development.

RELATING TO DENTAL SERVICES. Prohibits mutual benefit societies, health maintenance organizations, and dental service organizations from requiring dentists who provide services to their subscribers to accept fees set by a plan for any services, except for covered services.

RELATING TO VITAL STATISTICS FEES. Raises the fees associated with vital statistics certified copy and expedited and correction services.

RELATING TO MARRIAGE LICENSE FEES. Raises the marriage license fee.

RELATING TO TAX CREDITS. Provides a tax credit to any person who purchases a specific safety helmet approved for use by a person sixty-two years of age or older.

RELATING TO DISPOSITION OF DEAD HUMAN BODIES. Repeals the authority of the department of health to deliver or distribute an unclaimed dead human body to a university, hospital, or institution for medical educational and research purposes, and clarifies the responsibilities of the person who has possession, charge, or control of an unclaimed dead human body with respect to gathering and providing information to state agencies.

DECISION MAKING TO FOLLOW
Persons wishing to offer comments should submit testimony at least 24 hours prior to the hearing with a transmittal cover indicating:
· Testifier's name with position/title and organization;
· The Committee the comments are directed to;
· The date and time of the hearing;
· Measure number; and
· The number of copies the Committee is requesting.
While every effort will be made to copy, organize, and collate all testimony received, materials received on the day of the hearing or improperly identified or directed to the incorrect office, may be distributed to the Committee after the hearing.
Submit testimony in ONE of the following ways:
PAPER: 2 copies (including an original) to Room 310 in the State Capitol;
FAX: For comments less than 5 pages in length, transmit to 586-6281(for Oahu) or 1-800-535-3859 (for Neighbor Islanders without a computer to submit testimony through e-mail or the Web);
EMAIL: For comments less than 5 pages in length, transmit to HLTtestimony@Capitol.hawaii.gov ; or
WEB: For comments less than 10MB in size, transmit from http://www.capitol.hawaii.gov/submittestimony.aspx.
Testimony submitted will be placed on the Legislative Web site after the hearing adjourns. This public posting of testimony on the Web site should be considered when including personal information in your testimony.
If during the course of the hearing it appears that there will not be sufficient time to allow everyone to be heard, the chairperson reserves the right to limit the remaining testifiers to three minutes. This three minute policy will be enforced to ensure fairness and adequate consideration to all bills.
If you require special assistance or auxiliary aids and/or services to participate in the House public hearing process (i.e., sign or foreign language interpreter or wheelchair accessibility), please contact the Committee Clerk at 586-6150 or email your request for an interpreter to HouseInterpreter@Capitol.hawaii.gov at least 24 hours prior to the hearing for arrangements. Prompt requests submitted help to ensure the availability of qualified individuals and appropriate accommodations.
