§328-17.7 Record of prescriptions. Every practitioner, pharmacist, or medical oxygen distributor, who compounds, sells, or delivers any prescribed drug to a patient or a patient's agent shall maintain records that identify:

(1) The specific drug product;

(2) The prescribing practitioner;

(3) The patient;

(4) The date of prescribing or filling; and

(5) The name of the practitioner, pharmacist, or medical oxygen distributor dispensing the drug.

No prescription shall be compounded, sold, or delivered unless the name of the person compounding, selling, or delivering the same, or the name of the practitioner prescribing the same, is appended to the prescription in full, and every prescription shall be preserved for a period of not less than five years.
